| WOUND
CARE AND MANAGEMENT
Patients experiencing high-acuity problem wounds with
impaired or delayed healing typically have multiple,
complex and interacting conditions. Factors influencing
healing and treatment include advanced age, limited
mobility, incontinence, malnutrition, and existing chronic
conditions such as diabetes and peripheral vascular
disease. The wounds themselves may be complicating factors
in the treatment of other high acuity conditions, including
cardiac, pulmonary, neuromuscular and renal diseases.
Complex problem wounds are not only troublesome but
can be life-threatening. They also prolong acute hospitalization,
preventing the patient's transition to home or lower
level of care facilities. Our approach to advanced wound
management provides a comprehensive continuum, shaped
by a focus on resolving acute issues and leading the
patient toward healing and wellness.
